Zara Qairina Unconscious But Still Breathing While Traveling
KOTA KINABALU, April 14 (Bernama) — An assistant medical officer told the Coroner’s Court here today that Zara Qairina Mahathir was found unconscious but still breathing normally.
Kinarut Health Clinic Assistant Medical Officer, Mohd Faiszal Md Saimi, who was the first medical officer to arrive at the scene, said the victim was found unconscious with a Glasgow Coma Scale (GCS) reading of 15/3 which is usually associated with serious brain injury.
He said that although the victim’s GCS reading was very low, vital signs including blood pressure, pulse, respiratory rate and oxygen readings remained within normal limits during the journey to the hospital.
“While in the ambulance, I continued to check the girl’s blood pressure, oxygen readings, pulse readings and breathing rate using a machine.
“At that time the blood pressure showed 108/83 mmHg (normal), the pulse rate was 82 per minute (normal), the respiratory rate was 20 per minute (normal), and the oxygen reading in room air was 98 percent (normal), he said when reading the witness statement before Coroner Amir Shah Amir Hassan.
The 69th witness, aged 40, admitted that he received an emergency call at 03.23 WIB regarding a female student who had fallen in the Tun Datu Mustapha Kebangsaan Religious High School (SMKA) dormitory, Kinarut, before rushing to the location with her colleagues using an ambulance.
Arriving at the location at around 03.45, the victim was found lying on the cement floor at the edge of the dormitory building, unconscious.
Mohd Faiszal said the victim was then given initial treatment including the installation of a cervical collar, oxygen assistance and other supportive care before being sent to Queen Elizabeth I Hospital for further treatment.
He said that from further examination, the victim did not show any reaction, including to light stimulation of the eyes, apart from not making any sounds or noises, and did not respond to painful stimuli when the trapezius squeeze test was carried out on the left shoulder.
He also informed that he did not administer any medication to the victims during the initial emergency treatment, including anticonvulsant medications such as Phenytoin.
Earlier, the court was told that post-mortem laboratory tests detected the presence of the anticonvulsant drug, Phenytoin, in the victim’s kidney tissue.
When asked by the legal team for the victim’s mother, led by Shahlan Jufri, about the drug Phenytoin, Mohd Faiszal said that the drug belongs to class B which can only be given with the permission of a medical officer.
He said Phenytoin is usually used as a treatment option in cases of seizures that cannot be controlled with the first drug such as Diazepam.
“But for cases of seizures, the first choice is Diazepam. Phenytoin is the last drug choice for cases of uncontrolled seizures,” he said.
He said that the medicine is available in the form of tablets, capsules or injections, but for the general public it can only be obtained at pharmacies with a prescription from a doctor or medical officer.
Zara Qairina, 13, died at Queen Elizabeth Hospital on July 17 2025, a day after she was found unconscious in a drain near her school dormitory, at around 4am.
On August 13 2025, the Attorney General’s Office (AGC) ordered an examination to be carried out after studying the minutes of the police investigation, while on August 8 2025, the Attorney General’s Office issued an order to exhume the deceased’s grave so that an autopsy could be carried out.
The inspection process continues Thursday (April 16).
